Warfarin

The Institut national d’excellence en santé et en services sociaux (INESSS), in collaboration with a group of experts, has updated the medical protocol on anticoagulant therapy.

This protocol applies to individuals aged 18 and over who are using warfarin for anticoagulant therapy. The main changes include the following: an extension of the measurement interval of the international standardized ratio (ISR) in the presence of a stable ISR and of a clinical condition allowing the use of an extended interval; the number of warfarin adjustment tables that now incorporate all possible contexts of use (this number went from four in 2016 to two in 2019); and several adjustments related to the work that went into the production by INESSS of the two optimal use guides on the use of direct oral anticoagulants and warfarin.
