This document presents three approaches to improving health care practices. More specifically, it brings together the results of a selection of recent publications from the scientific and grey literature on the following approaches: 1) the learning systems approach, 2) the reflective approach and 3) audit and feedback (A-R).

These approaches have the same overall aim of improving the quality of health care, both for the patient and for the health care system, particularly with respect to front-line management of chronic diseases and multimorbid conditions.

This document provides a portrait of these aspects drawn from the literature review in the form of lessons learned and courses of action. It also presents a logical framework that integrates the three approaches with a view to facilitating their implementation.
