Workshop Content

  • Basic training in the principles, methods and tools used for continuous professional practice improvement, in the context of fulfilling a population-based responsibility and of a partnership of care and services to users and their loved ones.
  • A presentation of best practices and of the tools and services available for the targeted chronic disease.
  • A comparative review of:
    • health status indicators for the population of the area serviced;
    • care and service use indicators for users who have the targeted chronic disease.
  • A group reflection aimed at analyzing professional community practices:
    • a critical examination of clinical and organizational practices, with a special focus on interprofessional practices and on partnerships with users;
    • a review of the gaps between the norms/standards and current practices.


  • Identification in a team setting of potential avenues of action to improve the continuum of care and services provided to the public.
  • Development of an action plan for the implementation of priority actions.
