- Basic training in the principles, methods and tools used for continuous professional practice improvement, in the context of fulfilling a population-based responsibility and of a partnership of care and services to users and their loved ones.
- A presentation of best practices and of the tools and services available for the targeted chronic disease.
- A comparative review of:
- health status indicators for the population of the area serviced;
- care and service use indicators for users who have the targeted chronic disease.
- A group reflection aimed at analyzing professional community practices:
- a critical examination of clinical and organizational practices, with a special focus on interprofessional practices and on partnerships with users;
- a review of the gaps between the norms/standards and current practices.
- Identification in a team setting of potential avenues of action to improve the continuum of care and services provided to the public.
- Development of an action plan for the implementation of priority actions.
